Meet Ewa Bak, MD
Dr. Bak has been practicing medicine for 25+ years as a board certified internist, focused on patient care and building long term relationships. A graduate of the University of Notre Dame, she pursued her medical training at the Poznan University of Medical Sciences in Poland, where she was able to expand on her bilingual upbringing. After completing her residency training at the University of Kentucky, she remained in Lexington and established a successful Internal Medicine practice, also serving as track physician at Keeneland Race Track. In 2012, Dr. Bak and her family relocated to the Cleveland area, where she has continued as a well respected primary care physician, with repeated ratings as Top Doc by Cleveland Magazine.
Dr. Bak is married with 3 children and 2 Great Danes. She is passionate about Cleveland sports, music, Notre Dame football, and spending time with family and friends.

Direct Primary Care (DPC) is a simple model where you pay a flat monthly membership directly to your doctor — no insurance middleman in the exam room. That means longer, unhurried visits, real access when you need it, and a genuine relationship with a physician who actually knows you.
Your membership covers comprehensive primary care with no per-visit copays: annual physicals, same-day and illness visits, virtual visits, chronic condition management, and in-office procedures. One predictable price, billed monthly, so your care is never gated by a fee.
Couples and family members each receive 10% off their membership. Prefer to pay for the full year up front? That earns another 10% discount. You can combine relationship and annual savings to make membership even more affordable for your household.
Most members keep a traditional insurance plans or major medical policy for the things DPC does not cover — lab work, imaging studies, specialist visits, hospitalizations, surgeries, and emergency care. Think of your membership as covering everyday, personal primary care, while insurance handles the bigger, unexpected events.
Because we deliberately limit our practice size, same-day and next-day visits are the norm rather than the exception. You will never wait weeks to be seen for an acute concern, and appointments run on time — no crowded waiting rooms.
Yes. Many concerns can be handled from the comfort of home through secure virtual visits, saving you a trip when an in-person exam is not necessary. If a hands-on visit is needed, we will bring you in right away.
